Five/Six Person Team Rebate Forms

Use one of the attached forms:

FORM A – Full Year Payments

• Use this form if ALL members have paid full year fees.
• If any team member has only paid for a half year, the team must use Form B.

FORM B – Half Year Payments

• Use this form, if ONE or more team members have paid for a Half Year.
• You can complete another one in January if you sign up another 5 or 6 person team for the second half.
